Parental Consent

Parental/Guardian Consent for Members Under 18

As the parent/legal guardian of the minor named in a membership application, you acknowledge and agree to the following:

  1. Consent to Participate
    You give permission for your child to participate in all football activities organised by Glaslough Villa FC, including training, matches, and club events, in accordance with the club’s policies and procedures.

  2. Code of Conduct Agreement
    You have read and understood the Glaslough Villa FC Code of Conduct. You agree to support and uphold the standards of behaviour expected of players, parents, and spectators. You will ensure that your child understands and adheres to this Code of Conduct.

  3. Child Welfare and Safeguarding
    You understand that Glaslough Villa FC is fully committed to safeguarding the wellbeing of all its members in line with the Children First Act 2015, the FAI Child Welfare and Safeguarding Policy, and relevant Irish child protection legislation. You consent to your child being supervised and cared for by appointed club volunteers and coaches who have been Garda vetted and trained in safeguarding procedures.

  4. Medical Treatment
    In the event of illness or injury, you authorise the club’s coaches or officials to obtain any necessary medical assistance for your child, including the administration of first aid, and if necessary, to contact emergency medical services. 

  5. Transport and Photography
    You consent to your child being transported to and from games and events by appointed club volunteers, where necessary. You also consent (or opt-out where applicable) to your child’s image being used in club communications, including social media, the club website, and promotional materials, in line with the club’s media and photography policy.

  6. Data Protection
    You acknowledge that the club will collect and process personal data about you and your child for the purposes of club administration, in accordance with GDPR and the club’s privacy policy. Data will be stored securely and used only for legitimate club purposes.

  7. Liability
    You understand that while the club will take every reasonable measure to ensure the safety and welfare of members, participation in sport carries inherent risks. You agree that Glaslough Villa FC, its officers, coaches, and volunteers are not liable for any injury, loss, or damage unless caused by negligence or breach of duty.
